Logo Our skilled nursing facilities, assisted livings and home health agencies are working to reduce the impacts of COVID-19. We share the concerns of our residents and their families, our healthcare workers, and the communities they serve. We are carefully following the recommendations from the Centers for Disease Control and Prevention (CDC) to help reduce the potential spread of COVID among our staff and residents. We are also monitoring updates and revised recommendations made by the CDC. In addition, we monitor the guidance from the Centers for Medicare and Medicaid Services on this ever-changing situation. The leadership team at each facility is dedicated to doing all they can to minimize the impact of this pandemic in their buildings and communities. We will respond and adapt as the situation evolves.

The goal throughout the pandemic has been infection prevention and control while delivering quality care to the residents we have the privilege to serve.

Our Approach

We understand that moving from a familiar environment to a new community is a big change. We’re here to help you every step of the way.

Before you move in, we’ll develop a personalized health and wellness plan that includes a pre-admission appointment with your physician. The plan also includes your individual preferences, lifestyle interests and goals. Once you arrive, our transition team helps you thrive in your new home with services that include:

  • Medication management
  • Assistance with bathing and dressing
  • Wound care
  • Coordinated services with outside healthcare professionals
  • Recreational and social programming
  • Housekeeping, laundry service, and apartment maintenance
  • Transportation coordination
  • Three meals a day and snacks
  • Therapy services, including: physical, occupational, and speech

Additionally, there are options for utilizing in-house services such as:

  • Dental
  • Audiology
  • Podiatry
